 Registration
At Guangzhou CanAm International Medical Center, registration
is an one time process. Registration is for a new patient to establish
his/her medical record in the medical facility, and is completed
through the receptionist, nurse and doctor. We advise new patient
to come 15 minutes early before the scheduled appointment for registration.
The documents needed for registration:
Personal passport
An I.D. to identify your employment status
Name Card
Insurance Card (if there is one)
Appointment
We encourage patients to make an appointment with our doctor before
scheduling his/her visit to Guangzhou CanAm International Medical
Center. This appointment enables patient to see their doctor at
the specified time. Without an appointment, a walk-in patient may
not be able to see the doctor of their choice and the patient may
have to wait, except in an emergency situation.
After making an appointment, for follow-up, patients need to bring
the following documents to the Medical Center.
Personal photo I.D.
Insurance Card (if there is one)
Medical record number with Guangzhou CanAm International Medical
Center.
Membership Card with Guangzhou CanAm International Medical Center.
